Maybe a feature you paid for simply doesn’t work on Tuesdays. Your instinct is probably to blame carelessness, or a rushed team, or some junior developer who pushed to production on a Friday afternoon. That instinct is almost always wrong. The uncomfortable truth is that for most software companies, shipping with known bugs is not an accident. It is a calculated, defensible, sometimes brilliant business decision.

So companies should just fix everything before shipping, right? Here’s where it gets interesting. Achieving true zero-defect software is not a matter of trying harder. It’s computationally intractable at scale. A moderately complex application with 500,000 lines of code has an effectively infinite number of possible execution paths. Testing all of them is not a resource problem, it’s a logical impossibility. Even with 100 engineers running automated test suites around the clock, you are sampling the possibility space, not covering it.

This means every software team is already making triage decisions. The real question is never “should we ship bugs” but “which bugs are acceptable to ship.”

The Bug Triage Calculus

Here’s how a senior engineer actually thinks about a pre-release bug. Imagine you find a race condition (a bug where two processes interact in an unexpected order, causing unpredictable behavior) that crashes the app for roughly 0.3% of users under a specific set of circumstances. The fix requires refactoring a core module that three other features depend on. Estimated time: two weeks. Your release window is in four days.

You now have a decision tree that looks something like this:

  • Probability of user encountering this bug: low
  • Severity if they do: moderate (crash, not data loss)
  • Cost to fix pre-release: two weeks of two engineers
  • Cost to fix post-release: one week sprint after shipping, affecting far fewer users than the two-week delay would cost in lost revenue

In this scenario, shipping the known bug is often the correct call. Not the comfortable call. The correct one. The math works out to: the cure is worse than the disease, at least right now.

What “Done” Actually Means

There’s a concept in product development called the Minimum Viable Product, or MVP. The original definition, from Eric Ries, was precise: the smallest version of a product that lets you test a hypothesis with real users. Somewhere along the way, MVP became shorthand for “we shipped it before it was ready,” which is a perversion of the original idea but also, accidentally, still strategically sound in certain contexts.

Real user behavior is the only data that matters at scale. No amount of internal QA (quality assurance testing) replicates the diversity of devices, network conditions, accessibility needs, and usage patterns that actual users bring. This is the same logic behind why most billion-dollar apps launched with only three core functions. Constraint forces prioritization, and real-world feedback forces accuracy.

When a company ships a slightly buggy product, they are, in effect, running a distributed test across their entire user base. The bugs that matter rise to the surface fast. The bugs that nobody notices or nobody cares about get deprioritized. This is brutal and utilitarian, but it produces better signal than any internal testing environment can.

The Organizational Pressure Nobody Talks About

Software doesn’t just fail because of technical complexity. It fails because of people under pressure. Release dates are tied to marketing campaigns, investor announcements, seasonal purchasing windows, and competitive responses. The decision to ship on a known-buggy build is often made at a level far above the engineers who found the bugs.

This creates an interesting accountability gap. The engineer files the bug report. The PM (product manager) triages it to the next sprint. The VP approves the release. The user gets the crash. Nobody in that chain made an obviously wrong decision, and yet the outcome looks like negligence from the outside. This is a systems problem, not an individual failure, which is precisely why it persists across companies, cultures, and org sizes.

It also connects to something worth thinking about: digital transformation projects fail at an 84% rate often because companies are solving the wrong problem. The wrong problem, in this case, is treating bugs as a purely technical issue when they’re actually an organizational and economic one.

The Part Where the User Actually Matters

None of this means companies shouldn’t try to ship quality software. The calculus changes dramatically depending on what the bug affects. A crash in a note-taking app is annoying. A data corruption bug in medical records software is catastrophic. A payment processing error is a legal liability. The acceptable bug threshold scales inversely with the stakes.

The best companies are explicit about this. They maintain severity tiers (P0 through P3, or critical through low), they define what “shippable” means for each product category, and they treat that definition as a living document that gets revisited as the product matures. The worst companies treat every release as an implicit promise of perfection, fail to deliver it, and then scramble to explain why.

As a user, understanding this calculus doesn’t mean you should accept broken software passively. It means you’re better equipped to know when a bug is evidence of a genuine quality problem versus a rational triage decision that happened to affect you. Those two things require completely different responses, from both users and the companies building the products they depend on.
